By documentation I think many of us would mean a newspaper article with the
name of a specific school (town and state given). If a complaint were filed
it might well be public record (although the student would not be named),
particularly if legal action is required. Most states have some sort of open
meeting laws, so if someone were to go before a school board to complain on
such a topic most local newspapers would cover it. If a teacher is fired, there
is usually some sort of public record - and in this case I doubt either side
would keep it quiet.
 
I realize when it's family you're sure of your facts, but a good reference,
of the kind you'd like to see in a scientific or scholarly article, would be
most helpful.
